Care Coordinator

About the role

The Care Coordinator supports children with developmental, physical, or emotional disabilities through assessment, service planning, and coordination within the Children's Long Term Support (CLTS) Waiver Program.

Responsibilities

  • Conduct functional assessments and develop individualized service plans with families and providers.
  • Authorize and coordinate services aligned with care plans.
  • Facilitate family-centered team meetings and maintain accurate documentation.
  • Collaborate with service teams, participate in staff training, and testify in legal proceedings when needed.
  • Promote client independence, provide trauma-informed, family-centered services, and utilize evidence-based practices.
  • Complete time reporting accurately and consistently.

Qualifications

  • Bachelor's degree in a health or human services field (e.g., Social Work, Psychology, Counseling).
  • At least 1 year of experience working with children in the target population.
  • Strong communication, organizational, and documentation skills.
  • Proficiency with electronic health records and computer applications.
  • Valid driver's license, reliable transportation, and ability to respond in crisis situations.
  • Bilingual fluency in Spanish is preferred but not required.

Work Environment

The role involves community-based travel, occasional overnight stays, and a supportive work environment.
