Responsibilities

  • Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
  • Effectively manage a tiered caseload, while tailoring services to individual needs.
  • Similar responsibilities as a Care Manager.
  • Completes enrollment and eligibility documentation.
  • Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.
  • Commits to a respectful, just, and supportive environment for individuals and team members aligning with the organization’s commitment to inclusion and belonging.

Requirements

  • Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
  • Minimum of a Bachelor's degree is required by OPWDD in NYS
  • Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager, etc.
  • Valid driver's license

Schedule

Full-time Monday - Friday 8 Hour Shift No nights Hybrid. Hybrid Schedule Care Coordinators are required to attend three onsite days per week during the first 90 days to support onboarding and integration. After successfully completing this period, team members may transition to a hybrid schedule for added flexibility. Supervisors may extend the on-site requirement if needed.
