About the role
Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Early Intervention program.
Responsibilities
- Partner with families, service providers, and community resources to support child development.
- Conduct functional assessments and develop individualized service plans.
- Coordinate and facilitate services aligned with care plans.
- Support families through education, advocacy, and collaboration.
- Maintain accurate documentation in compliance with regulations.
- Lead family-centered team meetings and collaborate with internal teams and community providers.
- Manage a flexible schedule, including occasional evening or weekend hours, with remote work flexibility after training.
- Meet productivity and service delivery standards.
Qualifications
- Bachelor's degree in Social Work, Child Development, or related human services field.
- Preferred experience working with families with special needs.
- Valid driver’s license and reliable transportation.
Skills
- Strong communication, organizational, and documentation skills.
- Ability to work in home, office, and community settings, including responding effectively in crisis situations.
