About the role

Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Early Intervention program.

Responsibilities

  • Partner with families, service providers, and community resources to support child development.
  • Conduct functional assessments and develop individualized service plans.
  • Coordinate and facilitate services aligned with care plans.
  • Support families through education, advocacy, and collaboration.
  • Maintain accurate documentation in compliance with regulations.
  • Lead family-centered team meetings and collaborate with internal teams and community providers.
  • Manage a flexible schedule, including occasional evening or weekend hours, with remote work flexibility after training.
  • Meet productivity and service delivery standards.

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related human services field.
  • Preferred experience working with families with special needs.
  • Valid driver’s license and reliable transportation.

Skills

  • Strong communication, organizational, and documentation skills.
  • Ability to work in home, office, and community settings, including responding effectively in crisis situations.
