About the role

The Care Coordinator acts as the primary contact for families, facilitating access to services that promote safety and well-being. Responsibilities include conducting assessments, developing and authorizing individualized service plans, coordinating services, and maintaining accurate documentation.

Responsibilities

  • Perform functional assessments and re-assessments using approved tools
  • Develop and implement personalized service plans with families and providers
  • Coordinate and authorize services aligned with plans
  • Facilitate family-centered team meetings and collaborate with service teams
  • Maintain detailed client records and documentation
  • Participate in staff development and training activities
  • Testify at legal proceedings when required

Direct Service Expectations

  • Promote client independence
  • Deliver trauma-informed, family-centered services
  • Utilize evidence-based practices
  • Ensure accurate time reporting

Qualifications & Skills

  • Bachelor's degree in a health or human services field
  • At least 1 year of experience working with children in the target population
  • Strong communication, organizational, and documentation skills
  • Proficiency with electronic health records and computer applications
  • Valid driver's license and reliable transportation
  • Bi-lingual fluency in Spanish is preferred but not required
