Care Coordinator

About the role

We are seeking a Care Coordinator for Integrated Services for Behavioral Health (ISBH) in Athens, OH. This role is eligible for a $3,000 hiring bonus!

Responsibilities

• Uses a strengths based and solution focused mindset in all job functions and interactions internally and externally
• Develops, leads, and facilitates team meetings for the young person and their identified team
• Provides care coordination within the family’s home, community, or preferred location
• Pairs with the young person, family, natural supports, and other service providers to identify care coordination needs/services to ensure the best outcomes for young people and families
• Maintains required documentation to access and maintain OhioRISE benefits
• Maintains required certifications and completes all assigned trainings within identified timelines
• Continually acquires and maintains knowledge of community resources
• Attends mandatory in-person and virtual trainings and supervisions which will require travel
• Follows ISBH policies and procedures in maintaining confidentiality, professionalism, and incident reporting
• Utilizes databases and Electronic Health Record systems for accurate client documentation
• Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
• Travels within assigned service area

Requirements

• Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field is required
• Three years with a high school diploma or equivalent; two years with an associate degree or bachelor's degree; one year with a master's degree or higher
• Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment)
• Two years of experience in a coordinated supportive services or care coordination role that required high efficiency, organization, and attention to detail
• Experience working with people with autism spectrum disorders and developmental disabilities is preferred
• Experience in one or more of the following areas is required: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
• Able to effectively communicate through verbal/written expression required
• Able to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required
• Able to maintain confidential information is required
• Able to adhere to all safety, rules, regulations and requirements is required
• Able to bill Medicaid without restrictions is required
• Able to manage deadlines within a fast-paced, high-volume environment is required
• Able to operate in an Internet-based, automated office environment is required
• Able to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment is required
• Able to maintain a minimum of 100mbps for high-speed internet connection is required

Physical Requirements

• Prolonged periods of sitting at a desk and working on a computer is required
• Must be able to lift up to 15 pounds at times

Pay

The pay range for this position is $20.19 - $25.03 per hour based on experience, education, and/or licensure.
