Care Coordinator
About the role
Help patients successfully transition from inpatient psychiatric units, hospitals, and residential programs back into community care
Work closely with providers, nurses, and community partners to keep treatment plans aligned and effective
Improve patient safety by reviewing medications and coordinating follow-up care
Educate and support patients so they understand their care plan and feel confident moving forward
Build relationships across healthcare systems that strengthen mental health care in our region
Responsibilities

Requirements
Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing.
Preferred: Bachelor of Science in Nursing (BSN)
Preferred: experience in psychiatric/mental health nursing, or physician office or outpatient practice
Preferred: experience in care coordination or case management
Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers
Ability to communicate effectively with patients and families about treatment plans and care transitions
Demonstrated proficiency with technology including computers, electronic records, and common office equipment
Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes
