Care Coordinator

About the role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates care across the continuum, and collaborates with patients, families, and care teams to ensure effective discharge planning and care progression.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop individualized discharge plans
• Facilitate interdisciplinary rounds
• Address barriers to care
• Manage post-acute referrals
• Ensure accurate documentation
• Support care progression
• Promote resource utilization

Requirements

• Associate’s or Bachelor’s degree in Nursing
• Active RN license (single or multi-state)
• BLS certification
• At least 1 year of acute care nursing experience

Skills

• Excellent communication, assessment, organizational, and problem-solving abilities
• Ability to work under stress and handle change
• Collaborate effectively with care teams and families
• Knowledge of healthcare regulations and community resources
