About the role
We are seeking a registered nurse (RN/BSN) to serve as a Care Coordinator in our state-of-the-art long-term care facility. The ideal candidate will have strong clinical and assessment skills, experience coordinating resident care, and a passion for improving the lives of residents through evidence-based practice.
Responsibilities
- Coordinate resident care plans and assessments
- Demonstrate strong leadership, communication, and team-building skills
- Collaborate with care partners to build rewarding professional relationships
Requirements
- Current RN license (BSN preferred)
- Long-term care experience (preferred but not required)
- Dementia care experience (preferred but not required)
- MDS experience and certification (preferred but not required)
- Annual influenza vaccination or facility-approved exemption
- COVID-19 vaccinations highly encouraged
