Description Job Summary: The Care Coordinator serves as the primary point of contact for care coordination needs of individuals seeking services at the ELC. The Care Coordinator assists individuals in navigating access to whole-person care (behavioral and physical), facilitates screening and assessment tools, provides referral and coordination for internal and external health needs, and helps to engage individuals in recommended care. The Care Coordinator works directly with individuals experiencing behavioral health challenges, as well as with individuals' supports, internal ELC team members and external community agency partners. This position reports to the Quality Assurance Director or their designee. Major Job Responsibilities Welcomes individuals to ELC through exceptional customer service, to help provide information about ELC, the services we offer and the community services responsive to their needsAssists individuals in smooth transitions to different types/levels of careAssists individuals in completion of required documentationAssists in completion of client health measures (e.g., blood pressure, height, weight)Responsible for data entry and monitoring through excel spreadsheets and EHRMonitors assigned CCBHC (Certified Community Behavioral Health Center) outcomes and coordinates with internal and external providers to increase frequency of desired outcomesCoordinates care among providers and provides necessary follow-upDocuments activities in individual’s electronic health recordsMaintains confidentiality of all employee and client informationCompletes other duties as requiredCompletes all required trainings as assignedComply with ELC policies and procedures Requirements Education and/or Experience: Graduation from High School or the equivalent, some college preferredMinimum 3 years clerical or general office experience/training required, medical office experience a plusMust possess a compassionate attitude and present a professional demeanorFamiliarity with and/or lived experience with behavioral health challenges is preferred Knowledge, Skills, And Abilities Ability to communicate effectively orally, digitally and in writingAbility to perform tasks accurately, effectively, and in a timely mannerAbility to perform basic mathematics calculations accuratelyAbility to understand and follow oral and written instructionsAbility to successfully complete required trainingsAbility to establish and maintain effective working relationships with service recipients, other employees, and the publicHas a demeanor that is pleasant, friendly and helpful to visitorsWillingness and ability to support a diverse population of clientsAbility to safely perform major job responsibilities with or without reasonable accommodationsPossesses and supports ELC values of integrity, respect, quality, professionalism, and innovation
