About the role

The Care Coordinator supports children aged 0–3 with developmental delays or disabilities and their families through a family-centered, trauma-informed approach in the Birth to Three Program.

Responsibilities

  • Conduct functional assessments and develop individualized service plans
  • Coordinate and facilitate services and family team meetings
  • Partner with families, service providers, and community resources to promote child development
  • Support families with education, advocacy, and collaboration
  • Maintain accurate documentation in compliance with regulations
  • Collaborate with internal teams and community providers
  • Manage a flexible schedule based on client needs, including occasional evening or weekend hours
  • Meet productivity and service delivery expectations

Qualifications

  • Bachelor’s degree in Social Work, Child Development, or related field
  • Preferred experience working with families with special needs
  • Valid driver’s license and reliable transportation
  • Strong organizational, communication, and documentation skills
  • Ability to work in home, office, and community settings
  • Comfort in crisis situations and physical activity involved in travel
