About the role
The Care Coordinator supports children aged 0–3 with developmental delays or disabilities and their families through a family-centered, trauma-informed approach in the Birth to Three Program.
Responsibilities
- Conduct functional assessments and develop individualized service plans
- Coordinate and facilitate services and family team meetings
- Partner with families, service providers, and community resources to promote child development
- Support families with education, advocacy, and collaboration
- Maintain accurate documentation in compliance with regulations
- Collaborate with internal teams and community providers
- Manage a flexible schedule based on client needs, including occasional evening or weekend hours
- Meet productivity and service delivery expectations
Qualifications
- Bachelor’s degree in Social Work, Child Development, or related field
- Preferred experience working with families with special needs
- Valid driver’s license and reliable transportation
- Strong organizational, communication, and documentation skills
- Ability to work in home, office, and community settings
- Comfort in crisis situations and physical activity involved in travel
