Care Coordinator
About the role
At Atlas Oncology Partners, we believe every patient deserves personalized support as they navigate the complex cancer care system. We are seeking a dedicated Care Coordinator to serve as a consistent, non-clinical point of contact for patients and families throughout the cancer journey. This role is designed to reduce barriers to care, improve access to timely services, and provide practical and emotional support through close coordination with patients, providers, and interdisciplinary team members. - Assist with scheduling and follow-up appointments across the oncology continuum (e.g., surgery, infusion, radiation)
- Support connection to support groups, counseling, social work, and other internal or community-based services
- Establish trusting relationships with patients and caregivers through regular outreach and touchpoints
- Participate in interdisciplinary meetings and escalate patient concerns or needs appropriately
- Ensure patients complete key milestones, such as diagnostics, consults, and treatment starts
- Document patient interactions and support activities in the EMR clearly and in a timely manner
- Work closely with RN Care Coordinators, Social Workers, and Clinical Teams to ensure seamless, wraparound care
- Support in phlebotomy and vaccinations, as needed
