Qualifications
- BA/BS in related field with two (2) years of qualifying experiences required.
- An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered;
- Experience working with target populations strongly preferred.
Responsibilities
- Lead a team of patient navigators and patient assistant to ensure care coordination services are provided to all clients assigned to meet clients’ needs for better health outcome.
- Provide day-to-day supervision to the patient navigators and patient assistant assigned by weekly schedule monitoring, chart review, face-to-face case discussion, and performance reviews.
- Responsible for implementation of service/care plan, supported by Patient Navigators.
- Conduct outreach & recruitment for Apicha CHC to promote the Agency services.
