About the role
Care Coordinator for a Birth to Three Program supporting children aged 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Conduct assessments.
- Develop and implement family-centered service plans.
- Coordinate services and provide education and advocacy.
- Facilitate team meetings and ensure accurate documentation.
- Collaborate with families, service providers, and community resources to support child development and family needs.
- Maintain flexibility to adapt to client schedules and meet productivity expectations.
Qualifications
- Bachelor’s degree in Social Work, Child Development, or a related field.
- Experience working with families with special needs preferred.
- Valid driver’s license and reliable transportation.
- Strong communication, organizational, and documentation skills.
- Ability to work in diverse settings and respond to crisis situations.
Work Environment
Community-based role involving travel, home visits, and interaction in various environments, with moderate physical activity required.
