About the role
Care Coordinator for the Birth to Three Program supporting children aged 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Partner with families, service providers, and community resources to support child development.
- Conduct functional assessments and develop individualized, family-centered service plans.
- Coordinate services, facilitate team meetings, and support families through education and advocacy.
- Maintain accurate documentation and meet productivity expectations.
- Adapt schedules to client needs, including occasional evenings or weekends.
- After training, enjoy remote work flexibility with mandatory in-office meetings biweekly.
Qualifications
- Bachelor’s degree in Social Work, Child Development, or a related field.
- Preferably one year of supervised experience with families with special needs.
- Valid driver’s license, reliable transportation, and strong communication, organizational, and documentation skills.
- Ability to work in diverse settings and respond appropriately to crises.
Work Environment
- Community-based with daily travel, moderate physical activity, and exposure to various environments, including home and community settings.
- May include occasional overnight stays.
