Care Coordinator and Scheduler

About the role

One of the most unsung heroes in any home care agency is the Care Coordinator. At Phoenix Home Care & Hospice we recognize and celebrate our Care Coordinators as one of the most valuable, high-leverage roles on our team. As a Care Coordinator, you serve as the Communication Hub, the Emotional Support Co-Worker, and the Air-Traffic Controller for our nurses and caregivers in the field.

Responsibilities

• Coordinate daily caregiver schedules based on client care plans and staff availability
• Support and communicate with caregivers working in the field
• Complete client openings in the field
• Respond to client and employee questions or concerns and assist with resolution
• Maintain accurate documentation, records, and reports
• Assist with quality initiatives, staff communication, and training support
• Provide professional and courteous communication with clients, families, and team members

Requirements

• Must be at least 18 years of age
• High school diploma or equivalent required
• Previous office or administrative experience required
• Strong communication and organizational skills
• Ability to multitask and stay organized in a fast-paced environment
• Professional, dependable, and team-oriented attitude

Pay

$18 to $21 per hour
