Care Coordinator (4320)
Responsibilities
Provides intensive, short term assessment, linkage, counseling, and advocacy services to adults, children, and families who are experiencing a behavioral health crisis, in vivo at a level of intensity necessary to promote individual empowerment and recovery and reduce the need for inpatient treatment; assists in the development and implementation of individual services plans; provides support services to care givers; completes required documentation as prescribed by licensure, Medicaid, managed care groups or other third party payers with regards to clinical services and medical records keeping; and complies with all regulations applicable to program services; works as part of the team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Requirements
- Bachelors degree in Psychology, Social Work, Counseling, Sociology, Criminal Justice, or other approved Human Services degree
- Valid driver's license and safe driving record
- Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request

Pay
ANNUAL SALARY RANGE $49,631 - $86,028, with a 5% increase for Qualified Mental Health Professional and shift differentials: $2.00 for 2nd Shift, $3.00 for 3rd Shift.
