Care Continuum Partner - Medical Assistant

About the Role

The Care Continuum Partner - Medical Assistant supports both patients and clinical practices within a regional healthcare setting. This role involves collaborating with care teams to ensure smooth patient flow, conducting workflows for care gap closures, referrals, and medication authorizations, and supporting care plan adherence through patient outreach via various communication platforms.

• Facilitates virtual rooming and telehealth appointment concierge calls.
• Provides patient education on tests, procedures, and preparation.
• Assists with intake for annual wellness visits, prior authorizations, and referrals.
• Supports practice empanelment and serves as a liaison for patient portal and medical record technology, functioning as a virtual care concierge.

Qualifications & Skills

• Technical or specialized diploma as a Medical Assistant.
• Minimum of 2 years patient interaction experience and 3 years within a complex health system.
• High attention to detail, excellent customer service, and effective communication skills.
• Proficiency with basic computer functions and ability to maintain confidentiality.
• Adaptability, critical thinking, and multitasking capabilities.

Preferred Qualifications

• Associate’s Degree in Medical Assisting.
• Experience with electronic medical records, scheduling software, and medical terminology.
• Certification as a Certified Clinical Medical Assistant (CCMA).

Physical Demands

• Ability to lift and carry 25 lbs.
• Frequent sitting and standing.
• Perform keyboard tasks.
• Engage in activities like kneeling, bending, and CPR as needed.
