Care Continuum Partner - Medical Assistant

About the Role

The Care Continuum Partner - Medical Assistant provides support across clinical settings, focusing on patient engagement both within and outside of practice walls. The role involves collaborating with care teams to facilitate pre-visit planning, care gap closures, referrals, and medication authorizations.

Responsibilities

• Assist with patient outreach via various platforms, including phone, patient portals, and virtual tools.
• Support virtual rooming and telehealth appointment concierge services.
• Provide patient education on test preparations and procedures.
• Manage visit prior authorizations, referrals, and assist with empanelment.
• Serve as a liaison for patient portal and medical record technology, functioning as a virtual care concierge.

Requirements

• Technical or specialized diploma as a Medical Assistant; associate's degree preferred.
• Minimum of 2 years’ experience interacting with patients and performing core MA functions; 3 years within a complex health system preferred.
• Strong attention to detail, customer service, and communication skills.
• Proficiency in basic computer functions and confidentiality maintenance.
• Ability to adapt to changing priorities and demonstrate critical thinking.

Preferred Qualifications

• Knowledge of electronic medical records and medical terminology.
• Certification as a Clinical Medical Assistant (CCMA).

Physical Demands

• Ability to lift 25 lbs.
• Frequent sitting and standing.
• Perform activities such as bending or kneeling as required by specific tasks.
