About the role

The Care Consultant Sr (IN, KY, OH) role is designed to foster long-term partnerships with providers and enhance patient outcomes through value-based care initiatives across Medicare, Medicaid, and Commercial lines of business.

Responsibilities

  • Conduct detailed evaluations of provider practices to assess their capabilities in value-based care and identify areas for improvement.
  • Obtain and analyze practice-specific cost and quality data and reports to inform targeted interventions and strategies.
  • Support the implementation of population health management, care coordination, and care management strategies for high-risk members.
  • Create and deliver tailored learning opportunities to assist providers in effectively implementing recommended interventions.
  • Advise executives on developing segment-specific strategies in areas such as financial performance, incentive programs, quality management, and operational improvements impacting provider success.

Requirements

  • A Bachelor's Degree (BA/BS) and minimum of 5 years of health care experience; or a combination of education and experience providing an equivalent background.
  • Experience in physician environment, practice transformation, quality improvement, ambulatory care settings, quality and efficiency metrics, population health, and care management structures.
  • Strong understanding of value-based care principles and practices, excellent analytical skills, and the ability to interpret complex data and make strategic decisions.
  • Experience with data analytics and electronic health records, proven consulting experience in the healthcare industry, and familiarity with Medicaid, Medicare, and commercial plans.
  • Experience with Salesforce or similar CRM platforms, effective communication and facilitation skills.

Preferred Skills, Capabilities And Experiences

  • Physician environment experience in practice transformation/quality improvement, ambulatory care setting quality and efficiency metrics, population health and care management structures, data analytics and electronic health records.
  • Clinical background and/or MBA.
  • Proven experience in consulting within the healthcare industry.
  • Experience with Medicaid, Medicare, and commercial plans.
  • Strong understanding of value-based care principles and practices.
  • Excellent analytical skills with the ability to interpret complex data and make strategic decisions.
  • Experience with Salesforce or similar CRM platforms.
  • Effective communicator and facilitator.
