Join an established team of 10 physicians and 7 APPsCall structure: NonePractice setting: OutpatientEmployed position within a nonprofit health systemBoard Certified in Acute CareEligible for state medical licensure and DEA registrationPerform comprehensive clinical assessments—including volume status evaluations and outpatient IV diuretic management—to optimize guideline-directed medical therapy (GDMT) for complex heart failure patients.Coordinate seamless post-discharge follow-up care and interdisciplinary treatment plans to strengthen continuity of care and prevent hospital readmissions.Serve as a clinical leader and mentor for Heart Failure Center RNs, collaborating closely with community cardiologists, pharmacists, and care coordinators to execute individualized patient care plans. Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Education Master's degree of Nursing required.
