Cardiac Care Coordinator – Manage Heart Failure for Homebound LA Seniors Company: All Seniors Foundation Location: Los Angeles, CA About Us All Seniors Foundation is a 501(c)(3) non-profit committed to improving the quality of life for older adults across Los Angeles County. we provide medical care, in-home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status. Role Overview As Cardiac Care Coordinator, you will lead our home-based cardiac care program for seniors with heart failure, atrial fibrillation, post-MI recovery, and other cardiovascular conditions. You will work with cardiologists, primary care, and home health teams to keep seniors stable at home. Key Responsibilities Conduct telephonic and in-home cardiac assessments including BNP, weight, edema, vitals trackingEducate seniors and families on heart failure self-management, medication adherence, and dietary sodiumCoordinate care between cardiologists, primary care, and home health teamsMonitor patients using remote cardiac devices (BP cuffs, weight scales, implantable monitors)Identify and respond to early decompensation signs to prevent hospital readmissionsDocument encounters in EHR and contribute to cardiac quality metrics Qualifications Current CA RN license + BSN preferred2+ years cardiac nursing (telemetry, cardiac stepdown, CHF clinic, or home cardiac care)AACN Cardiac Medicine certification (CMC) preferredStrong patient-education skills around medications and lifestyle changesACLS certification What We Offer Competitive salary ($80,000–$110,000)Mix of home visits, telehealth, and remote monitoring workCMC certification fees + cardiac CEUs fundedComprehensive benefits including health, dental, vision, 401(k)Mileage reimbursement; quality-over-quantity caseload How To Apply Submit your application through the form on this page.
