About the role
Pacific Health Group is dedicated to improving health outcomes by addressing social determinants of health and coordinating comprehensive community-based services through our CalAIM programs. The Community Health Worker (CHW) serves as a vital link between healthcare systems and vulnerable populations, supporting members through direct engagement, education, and coordination of services that improve overall health and stability.
Responsibilities
- Spend approximately 65% of your time in the field (homes, community sites, events)
- Conduct outreach, assessments, and direct member engagement
- Travel within assigned county to meet members and community partners
- Support health education, care navigation, and resource coordination
- Facilitate group sessions and community-based activities
- Document services and interactions in real-time
Requirements
- High school diploma or equivalent; Associate's or Bachelor's degree preferred
- Minimum of 2 years of experience in community health, social services, or related field
- Strong understanding of community resources and social determinants of health
- Excellent interpersonal, communication, and engagement skills
- Proficiency in electronic health records and basic computer systems
- Bilingual skills preferred (Spanish or other relevant languages)
- Ability to document in real-time and manage multiple systems effectively
- Ability to travel extensively within assigned county
- Must reside within assigned service area
- Ability to work occasional evenings or weekends for community events
- Ability to work in a hybrid, field-based environment
Qualifications
- Valid California Driver's License (Class C minimum)
- Maintain a personal, operable vehicle for daily business use
- Maintain current liability insurance that meets California's minimum legal requirements
- Pass a Motor Vehicle Report (MVR) background check prior to employment
