About the Role

Full-time Certified Nursing Assistant position available. This role involves providing routine daily nursing care and services to patients in accordance with their assessment and care plan, as directed by supervisors.

Responsibilities

• Record all entries in an informative and descriptive manner using only authorized abbreviations
• Report all changes in patients to the Nurse Supervisor/Charge Nurse promptly
• Report all accidents and incidents observed during the shift
• Ensure patient rooms are ready for admission (bed made, name tags up, admission kit available)
• Greet and escort patients to their rooms
• Perform all assigned tasks in accordance with established policies and procedures
• Follow work assignments and schedules
• Cooperate with inter-departmental personnel to maintain nursing services
• Create and maintain an atmosphere of warmth, personal interest, and calm throughout the unit
• Meet with shift nursing personnel regularly to identify and correct problem areas
• Report all complaints and grievances made by residents
• Notify the facility when late or absent from work
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals
• Participate in and receive nursing report upon reporting for duty
• Follow established policies concerning exposure to blood/body fluids
• Assist patients with daily dental and mouth care
• Assist patients with bathing (bedbath, tub, or shower bath)
• Assist patients with activities of daily living (ADLs)
• Keep patients dry by changing gown, clothing, and linen when wet or soiled
• Change bed linens and make beds, keeping linens tight to avoid wrinkles
• Put extra covers on beds as requested
• Maintain intake and output records as instructed
• Assist patients in preparing for medical tests (lab work, x-ray, therapy, dental)
• Assist patients in preparing for activity and social programs
• Transport patients to/from appointments, activities, and social programs
• Assist with lifting, turning, moving, positioning, and transporting patients
• Assist patients to walk with or without self-help devices
• Perform restorative and rehabilitative procedures as instructed
• Change dressings, bandages, and binders as instructed
• Weigh and measure patients as instructed
• Measure and record temperatures, pulse, and respirations (TPRs)
• Answer calls promptly and check frequently on patients unable to call for help
• Check each patient routinely to ensure personal care needs are being met
• Observe and report pressure areas and skin breakdowns to prevent bedsores
• Report injuries of unknown source, including skin tears
• Assist with application of slings, elastic bandages, and binders
• Turn all medications found in resident's room/possession over to the Nurse Supervisor
• Watch for and report changes in room temperature, ventilation, and lighting
• Prepare patients for meals (bathroom, hand washing, comb hair, raise bed, position tables, place bibs)
• Serve food trays and assist with feeding as indicated
• Assist patients with identifying food arrangements
• Record food/fluid intake and report changes in eating habits
• Keep water pitchers clean and filled with fresh water on each shift
• Perform after-meal care (remove trays, clean hands/face/clothing, bathroom, brush teeth, clean dentures)
• Check rooms for unauthorized food items
• Review care plans daily to determine changes in resident care routine
• Inform Nurse Supervisor of any changes in patient condition for care plan updates
• Ensure notes reflect that the care plan is being followed
• Maintain confidentiality of all patient care information
• Treat all patients fairly with kindness, dignity, and respect
• Provide all nursing care in privacy and knock before entering patient's room
• Report all allegations of resident abuse and/or misappropriation of property
• Honor patient's refusal of treatment requests and report to supervisor
• Keep nurses' call system within easy reach
• Immediately notify Nurse Supervisor of elopement
• Follow established safety precautions in all duties
• Keep patient's personal possessions off the floor and properly stored
• Keep floors dry and report spills immediately
• Store excess supplies and equipment in designated areas
• Wash wheelchairs, walkers, and other equipment as instructed
• Clean, disinfect, and return all patient care equipment to designated storage after each use
• Perform routine housekeeping duties relating to nursing care procedures
• Store all tools, equipment, and supplies before breaks or at end of work day
• Report all hazardous conditions and equipment to Nurse Supervisor immediately
• Report all safety violations and smoking regulation violations
• Report any communicable or infectious disease to Director of Nursing Services
• Follow established isolation precautions and procedures
• Follow established procedures for use and disposal of personal protective equipment
• Wear and use safety equipment (back brace, mechanical lifts) when lifting or moving residents
• Report missing/illegible labels and SDSs to supervisor
• Use only equipment trained to use and operate all equipment safely
• Report defective equipment and equipment/supply needs to Nurse Supervisor
• Attend and participate in scheduled training and educational classes to maintain certification
• Attend and participate in orientation programs and annual OSHA/CDC training
• Attend advanced directive in-service training programs
• Maintain Relias learning
• Wash hands before and after performing any service

Requirements

• Must be a licensed Certified Nursing Assistant in accordance with state laws
• Must be able to read, write, speak, and understand the English language
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, and government agencies
• Must possess the ability and willingness to work harmoniously with other personnel
• Must have patience, tact, a cheerful disposition, enthusiasm, and willingness to handle difficult residents
• Must be willing to seek out new methods and incorporate them into nursing practices
• Must be able to relate information concerning a resident's condition
• Must not pose a direct threat to the health and safety of others in the workplace
• Must be able to move intermittently throughout the work day
• Must be able to speak and write English in an understandable manner
• Must be able to cope with the mental and emotional stress of the position
• Must be able to see and hear or use prosthetics that enable these senses to function adequately
• Must function independently with flexibility, personal integrity, and ability to work effectively with residents, personnel, and support agencies
• Must meet general health requirements including a medical and physical examination
• Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people
• Must be able to lift up to 10 pounds

Schedule

• 12-hour shifts
• Works beyond normal working hours, on weekends and holidays, and in other positions temporarily when necessary
• Subject to call-back during emergency conditions (severe weather, evacuation, post-disaster)

• Works throughout nursing service areas (drug rooms, nurses' stations, resident rooms)
• Moves intermittently during working hours
• Subject to frequent interruptions
• Involved with patients, personnel, visitors, and government agencies under all conditions
• Subject to hostile and emotionally upset residents, family members, personnel, and visitors
• Communicates with nursing and other department personnel
• Subject to injury from falls, burns from equipment, odors, and reactions to dust, disinfectants, tobacco smoke, and other air contaminants
• Subject to exposure to infectious waste, diseases, and conditions including TB, AIDS, and Hepatitis B viruses
• May be subject to handling of and exposure to hazardous chemicals
