Bridge to Discharge Counselor - LPC or LCSW
About the Role
Brentwood Hospital is seeking a dynamic Bridge to Discharge Counselor to join our team of professionals dedicated to providing help and hope to individuals and families, supporting their mental health improvement. The counselor will provide individualized case management, psychotherapeutic services, and finalize discharge planning for various patient populations, including adults, adolescents, and children, as well as their families. This role is integral to our interdisciplinary team, ensuring the delivery of quality treatment aligned with our organization’s philosophy.
Responsibilities
- Develops and coordinates an individualized discharge plan for patients by utilizing treatment team and chart information to determine aftercare needs.
- Identifies and assesses family or community resources, such as group homes or mental health practitioners, to meet patients' aftercare needs.
- Serves as a member of the interdisciplinary team supporting the organization’s treatment program and philosophy.
- Provides case management and psychotherapeutic services to adult psychiatric patients, adult chemical dependency patients, adolescents, children, and their families.
Requirements
- Master’s Degree in social work, counseling, or a related field.
- Experience in an inpatient or outpatient mental health setting preferred.
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) required.
- Knowledge of the Louisiana behavioral health marketplace, insurance plans, target markets, and referral sources is preferred.
- Understanding of psychiatric and chemical dependency treatment, as well as age-specific programming.
- Demonstrated business development concepts, sales abilities, and competency in computer usage.
- Excellent written and oral communication skills.
- Skills in data collection, analysis, and interpretation.
- Adherence to deadlines and strong customer service skills.
- Willingness to travel, including overnight travel as required.
Schedule
Monday - Friday, 8:30 AM - 5:00 PM.
