Breast Health, RN Nurse Navigator
Full-time, day shift position in the Comprehensive Women's Health Center.
About the role
The Breast Health Nurse Navigator provides patient care coordination by serving as a liaison between the patient and the practitioners and services she may require. The Women’s Health Navigator acts as a patient advocate, provides patient and family education, and coordinates all aspects of care to assure the provision of seamless comprehensive services. This role plays a critical role in the patient experience, including community outreach, optimizing patient services, and working collaboratively with the team to create a patient-centered environment. The scope of navigation extends beyond the services provided within the Comprehensive Women's Health Center and is intended to coordinate care across the continuum in a comprehensive way. The role is an integral part of the organization's commitment to providing holistic, patient-centered care for women.
Responsibilities
- Assess physical, psychological, and educational needs of patients according to guidelines, standards of care, and individual needs.
- Assess patient care needs and/or changes in condition throughout treatments and procedures, and communicate findings to the provider.
- Coordinate plan of care for patients, including appointments, transportation, education, and follow-up treatments to ensure timely delivery of diagnostic and treatment services.
- Coordinate referrals to medical and radiation oncology, physical therapy, and other services as determined by providers.
- Provide and coordinate education and information to patients and families to ensure a seamless, continuous, and comprehensive continuum of care.
Requirements
- Graduation from an accredited school of nursing; BSN preferred.
- Current licensure as a Registered Nurse (RN) in Connecticut.
- Minimum 2-4 years’ previous experience in a healthcare-related field.
- Must obtain certification for CN-BN within 12 months of accepting this position, as required by NAPBC.
