Requirements
- Valid driver's license with clean motor vehicle report.
- Basic computer skills and experience with an EMR system preferred.
- Case Management experience in mental health or behavioral health is preferred.
- Educational Requirements: Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or Registered nurse; or A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years of experience in direct contact with mental health consumers; or A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.
Responsibilities
- Develop, coordinate, and implement services for consumers ages 6 to adult based on goal‑oriented service plans.
- Provide case management, service planning, and coordination with other agency providers to meet consumer needs and increase community stability.
- Link consumers with mental health or behavioral health diagnoses to resources and encourage, teach, and guide them to advocate for themselves.
- Complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact.
- Complete paperwork requirements from OMHSAS, Magellan Behavioral Health, Montgomery County, and Merakey in a timely manner.
- Conduct appointments with consumers in homes, community, and hospitals; be familiar with Montgomery County and able to access resources to meet client needs.
- Manage a caseload of 25‑30 clients, meeting with clients every 14 or 30 days according to matrix score while maintaining productivity standards.
- Attend monthly supervision meetings with manager and weekly team meetings.
- Work Monday‑Friday 9:00 am‑5:00 pm (shifts may vary
