Blended Case Manager

About the role

The Blended Case Manager is responsible for providing comprehensive support to families in need, integrating various services and resources to ensure holistic care.

Responsibilities

• Assess family needs and develop personalized plans
• Coordinate service delivery from multiple providers
• Maintain regular contact with families to monitor progress
• Advocate for families at community meetings
• Document all interactions and outcomes in a timely manner

Requirements

• Bachelor's degree in Social Work, Psychology, or related field
• At least 2 years of relevant experience in social services or related field
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite

Qualifications

• Valid driver’s license and reliable transportation
• Ability to work flexible hours including evenings and weekends
