About the role
The Blended Case Manager is responsible for providing comprehensive support to families in need, integrating various services and resources to ensure holistic care.
Responsibilities
- Assess family needs and develop personalized plans
- Coordinate service delivery from multiple providers
- Maintain regular contact with families to monitor progress
- Advocate for families at community meetings
- Document all interactions and outcomes in a timely manner
Requirements
- Bachelor's degree in Social Work, Psychology, or related field
- At least 2 years of relevant experience in social services or related field
- Strong communication and interpersonal skills
- Proficiency in Microsoft Office Suite
Qualifications
- Valid driver’s license and reliable transportation
- Ability to work flexible hours including evenings and weekends
