Blended Case Manager

About the role

The Blended Case Manager is responsible for providing comprehensive support to families in need, integrating various services and resources to ensure holistic care.

Responsibilities

• Assess family needs and develop personalized service plans
• Coordinate services from multiple agencies and providers
• Maintain regular contact with families to monitor progress and address challenges
• Provide emotional support and guidance to families
• Collaborate with other professionals to enhance service delivery

Requirements

• Bachelor's degree in Social Work, Psychology, or related field
• Minimum 2 years of relevant experience in social services or related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Pay
