About the role
The Blended Case Manager is responsible for providing comprehensive support to families in need, integrating various services and resources to ensure holistic care.
Responsibilities
- Assess family needs and develop personalized service plans
- Coordinate services from multiple agencies and providers
- Maintain regular contact with families to monitor progress and address challenges
- Provide emotional support and guidance to families
- Collaborate with other professionals to enhance service delivery
Requirements
- Bachelor's degree in Social Work, Psychology, or related field
- Minimum 2 years of relevant experience in social services or related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
Qualifications
- Valid driver’s license and access to reliable transportation
- Experience working with diverse populations
- Knowledge of community resources and services

Pay
Salary range: $40,000 - $50,000 annually
