Bilingual Youth Case Manager

Responsibilities

  • Intake, Assessment & Treatment Planning
    • Administer CALOCUS assessments to determine appropriate levels of care
    • Complete intake paperwork and conduct comprehensive mental health needs assessments
    • Develop individualized service, safety, and treatment plans for each client
    • Update treatment plans, risk assessments, and clinical documentation every six months or as needed
    • Develop and implement transition, discharge, and aftercare plans before services end
  • Service Coordination
    • Coordinate supportive services for clients and families
    • Communicate weekly with guardians and caregivers on treatment progress
    • Conduct at least one monthly in-person or virtual check-in with every family on your caseload
    • Participate in departmental staffing meetings and debrief with the BHT team
  • Client Advocacy
    • Advocate for client needs and ensure access to appropriate services
    • Assist families with tasks requiring extra guidance, such as AHCCCS appointments and crisis navigation
  • Facilitation & Community Engagement
    • Facilitate Child and Family Team (CFT) meetings
    • Attend quarterly school staff meetings to provide client progress updates
    • Build and maintain relationships with community resources and referral partners
  • Documentation & Compliance
    • Maintain accurate, up-to-date client records including case notes, service plans, and monthly progress reports in compliance with state regulations and company policy
  • Crisis Intervention
    • Provide crisis intervention and support to clients and families as needed

Requirements

  • Spanish language proficiency strongly preferred
  • Strong organizational, planning, and task management skills
  • Ability to maintain composure under pressure and adapt quickly
  • Empathy and skill in building relationships with families from diverse backgrounds
  • Clear verbal and written communication with clients, families, and clinical professionals
  • Critical thinking and problem-solving ability
  • Comfort with technology, including Google Workspace and Adobe PDF
  • Ability to work both independently and collaboratively as part of a team

Qualifications

  • Bachelor's degree in social work, psychology, or a related field preferred; relevant experience may substitute
  • Minimum of one year of experience (including internships or volunteer work) with children with mental health needs

Pay

Annual Salary: $40,000 – $45,000 per year

Monday – Friday, 8:00 AM – 5:00 PM (some slight scheduling flexibility is required for specific cases)
