Responsibilities
- Deliver care to complex patients using telehealth and an interdisciplinary approach
- Guide medical management utilizing the 5M framework
- Collaborate with the patient's healthcare team and support systems
- Perform comprehensive assessments and medication reviews for patients
- Provide telehealth support for other service lines if needed, compliant with NP licensure
- Maintain accurate patient records following legal and ethical standards
- Participate in weekend on-call rotation as the program expands
Qualifications
- Active RN and APRN or Physician Assistant license with multi-state licensure willingness
- Minimum 3 years as an APP in primary care, geriatrics, or related fields
- Proficient in telehealth and electronic health records
- Current BLS certification required
