Responsibilities
- Conduct comprehensive initial post-discharge assessments, including medication reconciliation
- Assess and manage new or worsening symptoms of patients during virtual urgent-care visits
- Adjust and reinforce care plans for patients in their 30-day post-discharge window
- Collaborate with interdisciplinary teams during TOC rounds to minimize readmission risks
- Encourage open communication within the team and support patient escalation processes
- Participate in clinical education and quality improvement projects for the team
- Adept to evolving service needs by supporting new clinical initiatives
Qualifications
- 5+ years of direct patient care experience focusing on geriatric populations with multiple chronic conditions
- Demonstrated ability to manage both acute and chronic patient health issues effectively in a cohesive continuum
- Strong independent clinical decision-making skills, especially in virtual care settings
- Proficiency in complex medication reconciliation, particularly for heart failure, COPD, and diabetes
Compensation
$130,000 – $170,000 annually
