Bilingual Spanish/English Active Primary Care APP (NP/PA)- Telehealth

Responsibilities

• Provide virtual primary care visits, including assessments, chronic disease management, and acute care interventions
• Diagnose and manage chronic conditions and develop individualized care plans for patients
• Evaluate and treat acute concerns within the scope of virtual care
• Order and interpret diagnostic labs and imaging, coordinating follow-up when necessary
• Manage medication prescriptions and ensure patient adherence and safety
• Educate and empower patients in managing their health through coaching and lifestyle modifications
• Identify and facilitate referrals to specialists or community resources as needed

Requirements

• Active Nurse Practitioner (APRN) license in good standing with multi-state licensure capabilities planned within 90 days of hire
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
• Minimum 2 years' experience in primary care, urgent care, or internal medicine, preferably in telehealth
• Active BLS certification upon hire
• Flexibility to accommodate varied shift times across patients' local time zones, including potential weekend shifts

Pay

$125,000 – $125,000 annually
