About the role
Advocate & Strategist: You will bring patient care plans to life—implementing, monitoring, and evolving them to meet the unique needs of those in your care. Vigilant Communicator: Beyond just "recording" data, you will be the expert voice for your patients, monitoring conditions and communicating vital updates to the team in real-time. Precision Execution: With a keen eye for detail, you’ll interpret and carry out physician and nursing orders, ensuring clinical accuracy at every turn. Bridge to Home: You’ll lead the discharge planning process from day one, collaborating across the care team to ensure your patients have everything they need for a safe and confident transition home.
Responsibilities

- Interpret and carry out physician and nursing orders, ensuring clinical accuracy at every turn.
- Lead the discharge planning process from day one, collaborating across the care team to ensure your patients have everything they need for a safe and confident transition home.
Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- One or more of the following required:
- American Board of Nursing Examiners (ABNE) Registered Nurse (RN) credential from the Tennessee Board of Nursing.
- Current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.
