About the role
Peace Healthcare Agency is seeking an experienced Patient Care Coordinator to join our growing home healthcare team. The role involves coordinating patient care and caregiver schedules, communicating with patients, families, caregivers, and healthcare professionals, managing schedule changes, call-offs, and staffing needs, following up on patient needs, concerns, and satisfaction, maintaining accurate documentation and schedules, assisting with intake and service coordination, and protecting patient confidentiality while following HIPAA requirements.
Responsibilities
- Coordinate patient care and caregiver schedules
- Communicate with patients, families, caregivers, and healthcare professionals
- Manage schedule changes, call-offs, and staffing needs
- Follow up on patient needs, concerns, and satisfaction
- Maintain accurate documentation and schedules
- Assist with intake and service coordination
- Protect patient confidentiality and follow HIPAA requirements
Qualifications
- Spanish-speaking is required
- Previous home healthcare/home care experience required
- Experience with patient care coordination, scheduling, intake, or staffing
- Excellent communication and organizational skills
- Ability to multitask in a fast-paced environment
- Proficient with computers and healthcare/scheduling software
- Professional, dependable, and patient-focused
Pay
$18–$25 per hour
