Key Responsibilities
- Perform comprehensive evaluations of patients, including physical exams, acute care, preventive care, and chronic disease management.
- Diagnose, treat, and manage a wide range of primary care conditions, including hypertension, diabetes, respiratory illnesses, and more.
- Provide care coordination for patients transitioning between healthcare settings, including hospital discharge follow-ups and post-acute care.
- Educate patients and families on maintaining proper health, preventive care, and managing chronic conditions.
- Provide urgent care services, including treatment for minor injuries, infections, and other acute medical issues.
- Manage and monitor value-based patients, ensuring compliance with care plans to achieve quality benchmarks.
- Utilize HEDIS measures to evaluate and improve patient care, including closing care gaps and reporting on quality metrics.
- Work as part of an interdisciplinary healthcare team to coordinate and optimize patient care.
- Collaborate with nurse practitioners, physician assistants, medical assistants, and administrative staff to enhance patient outcomes.
- Participate in case discussions and team meetings to develop comprehensive care plans.
- Engage in initiatives aimed at improving HEDIS scores and ensuring high-quality care for value-based patients.
Qualifications
- Education: Medical degree (MD or DO) from an accredited institution.
- Licensure/Certification: Valid medical license to practice as a physician in the state of Florida. Board certification in Family Medicine or Internal Medicine. DEA license and current CPR certification.
- Experience: Minimum of 2 years of experience in a primary care setting preferred. Experience managing value-based care patients and utilizing HEDIS measures for quality improvement. Familiarity with interdisciplinary team-based care and chronic disease management. Experience with diverse patient populations, including pediatric, adult, and geriatric care.
- Skills and Abilities: Strong clinical and diagnostic skills with a focus on evidence-based care. Excellent communication and interpersonal skills to build rapport with patients and team members. Proficiency with EMR systems (experience with Athena is a plus). Ability to work effectively in a fast-paced, patient-centered environment. Bilingual (English/Spanish) is highly favorable. Demonstrated ability to provide compassionate, patient-focused care.
