About the role
Mentor Maryland, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing various out-of-home situations. Our mission is to help youth gain stability, comfort, and confidence to overcome challenges and thrive.
Responsibilities
- Provide assessments of individuals and families in areas such as mental status, personality, behavioral response patterns, relationships, and family interactions.
- Propose a service or treatment plan based on the assessment of family needs and make practical suggestions for change.
- Implement activity and participation-based interventions and approaches using training materials, personal experience, and feedback from families and peers.
- Provide 24/7 coverage for all assigned individuals and families.
- Develop and implement a closure and aftercare plan for each family and individual served.
- Build and maintain positive relationships with personnel from referring/collateral agencies, guiding families in their interactions with these agencies and other community organizations as needed; provide referrals to services and resources on behalf of families.
- Ensure confidentiality, organization, and completeness of documentation and records; submit such documentation accurately and promptly to the supervisor.
- Travel up to 1 hour for families served.
- Participate in team meetings with peers and collateral team meetings as needed.
- Support peer development by addressing areas of concern for professional development and team performance issues.
- May provide training or field supervision of new or tenured peers; may review and edit peer records and documentation, marketing and education presentations, and training and development materials for the region or team meetings.
Requirements
- Bachelor's Degree in a related Human Service field required.
- One year of case management or behavioral health experience.
- Bi-lingual (Spanish-English) required.
- Specialized training required.
- Self-motivated and detail-oriented with the ability to multi-task.
- Excellent communication and analytical skills.
- Valid Driver’s license and reliable vehicle, including valid registration and insurance.
