About the role

The Case Worker is expected to work under the guidance of the Social Worker II. They will develop 1:1 relationships with the residents which will enable them to provide a range of services to the residents which enable the residents to establish and maintain safe and independent lives in the community.

Responsibilities

  • Maintain accurate and organized case files, records, and reports.
  • Write progress notes after each resident encounter.
  • Develop individualized service plans with residents.
  • Draft correspondence, notices, and reminders for residents as needed.
  • Complete data entry of resident progress in internal databases.
  • Aid residents with general inquiries or referrals to services.
  • Accompany residents to appointments or community activities as needed.
  • Help facilitate social and recreational activities for residents.
  • Assist in preparing materials for group sessions or workshops.
  • Carry a caseload of residents who present less challenging.
  • Participate in team meetings and trainings.
  • Assist social workers in emergency situations as directed.
  • Report urgent resident concerns promptly to supervising social workers.

Requirements

  • High school diploma or equivalent; associate or bachelor's degree in social work, Human Services, or related field preferred.
  • Interest in working with older adults and vulnerable populations.
  • Strong organizational, interpersonal, and communication skills.
  • Basic computer skills and familiarity with data entry.
  • Ability to work collaboratively in a team environment.
  • Ability and willingness to travel to multiple sites.

Qualifications

  • Bilingual English/Spanish preferred.
  • Experience in social services, supportive housing, or non-profit settings.

Skills

  • Good written and oral communication skills.
  • Good organizational ability, time management, and interpersonal skills.
