Responsibilities
- Conduct eligibility assessments for clients, adhering to specific program guidelines
- Verify that clients do not have other payer sources, such as Medicaid, Medicare, or third-party insurance
- Register clients into the CPCDMS and complete all necessary paperwork
- Perform data entry of eligible clients into our electronic system
- Maintain up-to-date and orderly eligibility files for seamless access and review
- Conduct renewal eligibility assessments as required by each program
- Confirm eligibility appointments and set initial service appointments once eligibility is determined
- Provide clients with a list of required eligibility documentation
- Participate actively in the Performance Improvement Program
- Promote effective working relations and collaborate as part of a dedicated team
- Attend all required safety training programs and adhere to safety protocols
