About the Role
As a Bilingual Community Outreach Representative at Personal Touch Home Care of N.Y. Inc, you will play a crucial role in connecting with our diverse community, promoting our services, and building relationships with patients and families. With a strong focus on personalized care, we are seeking a dedicated and compassionate representative who can effectively communicate our services to our bilingual audience.
Responsibilities
- Conduct community outreach and education programs to promote our services to patients, families, and healthcare providers.
- Build and maintain relationships with community organizations, health centers, and social services agencies to identify potential partnerships and referrals.
- Identify and address the social, emotional, and physical needs of patients and families, and provide personalized support and resources.
- Develop and maintain a strong understanding of our services, including home health and home care programs, to effectively communicate our offerings to the community.
- Collaborate with our multidisciplinary team to develop individualized care plans that meet the unique needs of each patient.
- Conduct language‑specific outreach and education programs to ensure that our bilingual patients and families receive the care and support they need.
Requirements
- Strong communication and interpersonal skills, with the ability to effectively communicate with patients, families, and community organizations.
- Bilingual in English and Spanish with strong reading, writing, and speaking skills.
- Ability to work independently and as part of a team to achieve goals and objectives.
- Strong problem‑solving and organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
