Bilingual Case Manager for Individuals With Disabilities

Job Description

This position involves coordinating in-home and community-based services for the elderly and people living with various types of disabilities. Key responsibilities include:

• Working Monday through Friday from 8 AM to 5 PM.
• Occasionally working different schedules to accommodate client needs, including direct care shifts and evenings/weekends.
• Performing on-call duties at least 4-5 times per year, including answering emergency phone calls from 7 PM to 7 AM Monday through Friday and all day Saturday and Sunday.
• Maintaining and growing a caseload.
• Scheduling shifts for clients and direct care staff.
• Orienting and training direct care staff with clients.
• Monitoring direct care staff performance.
• Holding staff meetings as needed and attending weekly department meetings.
• Conducting intake of new clients.
• Meeting with clients/families in their home at least quarterly.
• Attending meetings with other agencies involved in a client's care.
• Ensuring that all paperwork in client files is updated regularly.
• Documenting all significant conversations and/or correspondences with staff and clients.
• Maintaining client and agency confidentiality at all times.

Qualifications

• At least 18 years of age or older.
• Able to obtain a current CPR/FA certification.
• Able to obtain fingerprint clearance.
• Able to clear a DES background check.
• Able to obtain a current TB clearance card.
• The ability to work in a team environment.
• The ability to handle difficult situations with clients/families.
• The ability to work a flexible schedule.
• The ability to learn and operate UCPSA program software.
• The ability to work independently.
• The ability to lift and transfer clients (up to 50 lbs).
• Own/have regular access to and be able to operate a motor vehicle for the purpose of visiting and transporting clients, attending meetings outside of the office, etc.
• Proof of current registration and insurance of the vehicle must be kept on file with UCPSA.
• At least one year of direct care experience, personally or professionally, a degree in a related field, and/or case management experience.
• At least one year of teaching or related experience (paraprofessional, habilitation provider, registered behavior technician, etc.).
• Bilingual in English and Spanish is required.
