Responsibilities

• Support optimal member health outcomes by coordinating care, providing education, and facilitating access to appropriate healthcare services.
• Using care management tools and data analysis, assess member needs, identify risks, and develop and implement effective care plans.
• Collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits.
• Promote member engagement, informed decision-making, and healthy behavior changes through communication, coaching, and motivational techniques.
• Monitor care plan progress, document activities, and ensure compliance with regulatory standards, company policies, and quality management guidelines.

Requirements

• Bilingual (English & Spanish): Required. Must successfully pass both verbal and written language assessments.
• Travel Requirement: Must have reliable transportation and be willing/able to travel up to 75% of the time. Mileage is reimbursed per company policy.
• Location Requirement: Must reside in Homestead and Florida City, FL within zip codes 33033 and 33032.
• Experience: At least 1 year in a behavioral health or long-term care setting.
• Technical Skills: Proficiency in Microsoft Office Suite as well as computer use.

Preferred Qualifications

• Experience in discharge planning, case management, or managed care (MCO).
• Strong ability to multitask, prioritize, and adapt in a fast-paced environment.
• Advanced communication skills in both English and Spanish (verbal and written).

Qualifications

• Bachelor’s Degree or non-licensed Master’s level clinician required. Degree to be in behavioral health, human services (psychology, social work, marriage and family counseling).

• The Typical Pay Range For This Role Is $21.10 - $40.90
• This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
• This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
