This role reports directly to our Senior Vice President of Customer Experience and is a critical part of Epicured’s New York Medicaid Section 1115 Waiver program, supporting Medicaid members through screening, navigation, and connection to eligible services. Position Overview Epicured is hiring Social Care Navigators to support New York’s Medicaid Section 1115 Waiver program. In this role, you’ll engage Medicaid members, assess eligibility for Health-Related Social Needs (HRSN) services, and coordinate referrals through Epicured and community partners. This role, reporting to the VP of Member Experience, is ideal for someone who is organized, compassionate, tech-savvy, and comfortable working in a high-volume virtual contact center environment. Key Responsibilities Manage inbound and outbound member communications via phone and email, leveraging Five9 and various partner technology platformsConduct outreach to screen potential members, assess eligibility, and navigate members to service providers based on identified health-related social needsTrack and follow up on referrals to ensure service delivery and satisfactionMaintain accurate documentation in program platforms and internal systemsSupport members with program questions Comply with Medicaid, HIPAA, and NYS DOH requirements Qualifications Required High school diploma (Associate degree preferred)3–5 years of experience in customer service, call centers, social services, care navigation, or related fieldsExperience in a virtual contact center environment (Five9 preferred)Strong communication and member engagement skillsAbility to manage high-volume interactions in a compliance-driven setting Preferred Experience with Medicaid, Social Care Networks (SCNs), or health-related social needs (HRSN) programsBackground in healthcare, care coordination, patient support, or community-based services Multilingual candidates encouraged to apply (Spanish, French, Mandarin, Hindi) Schedule Availability between 9:00 AM–6:00 PM EST, Monday–Saturday, with weekly schedule assigned based on evolving business needs Compensation & Benefits Pay: $21–$23/hour, based on experience Benefits: 401(k)Medical, dental, and vision insuranceEmployee meal discounts Equal Opportunity Employer Epicured is proud to be an Equal Opportunity Employer.
