BH Navigator - Staff - Full-time - AAMC

Position Objective

To provide linkage to continuous and comprehensive services necessary for the promotion of optimal mental health and the prevention and treatment of psychiatric and substance use disorders to patients across the age spectrum. The clinician works collaboratively with medical and mental health providers, as well as community agencies and primary care providers.

Essential Job Duties

• Provides consultation and education to providers in the implementation and coordination of multi-disciplinary care.
• Provides referrals to the patient for outpatient mental health treatment and substance abuse treatment.
• Serves as a liaison between the patient and ancillary services, e.g., treatment centers or programs as well as serving as the direct contact to the patient.
• Refers patients to other resources or services in the community, such as support groups and recovery groups.
• Maintains a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
• Interviews and assesses patient using behavioral health knowledge and skills in making referrals.
• Negotiates process needs and facilitates appropriate change through collaboration with emergency department and information technology personnel.
• Develops and maintains detailed list of community resources for referral.
• Collaborates with Community Behavioral Health Navigator regarding community resources and accessibility of services.
• Tracks and reports monthly metrics for Bay Area Transformation Grant.

Education/Experience Requirements

• Master’s Degree in Counseling or Social Work required (Social Work preferred)
• Two years of experience in the field of behavioral health.

License/Certifications

• Working Conditions, Equipment, Physical Demands: There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.
• Physical Demands – Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

Pay Range

• $62,400—$94,494.40 USD
