Job Summary

Captures and oversees the plan of care and service delivery for behavioral health consumers across the continuum of illness; promotes effective utilization and monitoring of health care resources; collaborates with the interdisciplinary team to achieve optimal clinical outcomes.

Details of the position

  • Provides individual, family, group, and multi-family group therapy to patients with complex diagnostic presentations and treatment needs.
  • May provides clinical leadership to team of masters’ and bachelors’ level clinicians, CSWA candidates and MSW interns.
  • Affirms a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and their self-identified natural supports.
  • Evaluates and provides interventions to improve a patient’s cognitive, social, vocational, spiritual, and emotional functioning, utilizing natural and community supports when possible.
  • Performs urgent/crisis evaluations, interventions, and safety planning.
  • Together with consumer and natural supports, identifies key problems, strengths and resources to be addressed in the treatment plan.
  • May provide case management, coordination with other treatment agencies or outside support services, outreach, and engagement.

What you bring

  • Master's Degree Required: Social Work, Counseling, Psychology, or Marriage and Family Therapy.
  • Minimum of 3 years Required: Experience in behavioral health.
  • Experience in diagnosis and treatment of patients with complex mental health conditions.
  • Preferred: Experience provided clinical supervision to bachelors’ and masters’ level clinicians.
  • Required Upon Hire: Licensed Professional Counselor - Oregon for jobs based in Oregon or
  • Required Upon Hire: Licensed Clinical Social Worker - Oregon for jobs based in Oregon or
  • Required Upon Hire: Licensed Marriage & Family Therapist-Oregon for jobs based in Oregon.
  • Required Upon Hire: Basic Life Support.

Skills

  • Interpersonal skills to obtain and interpret information appropriate to patients’ needs and age as required for assessment range of treatment, and patient care (Required).
  • Proficiency with DSM-5 diagnoses and best practice treatment interventions for a variety of behavioral health conditions (Required).
  • Demonstrated ability to promote positive interpersonal relationships and patient/family centered work environment (Required).
  • Knowledge and adherence to Social Work Code of Ethics (Required).
  • Basic computer skills including Microsoft Office applications and documenting in electronic medical records (Required).
  • Effective prioritization skills (Required).

Working Conditions

  • Lifting Ability to move around area with occasional sitting.
  • Bend/ twist/ reach/ push/ pull occasionally.
  • Lift/Carry: Occasionally up to 5 lbs.

Environmental Conditions

  • Exposure to biohazard, body fluids and airborne particles.
  • Time spent on site in medical/hospital setting or in office environment.

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
