Bereavement Counselor, Hospice

Preference will be given to candidates with experience in a Home Hospice environment.

About the role

Assists with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care and in alignment with SCAH, mission, goals and objectives. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death. Bereavement counseling will ensure that the bereavement services reflect the needs of the bereaved.

Requirements

• Master’s degree in social work or related field (equivalent experience will be accepted in lieu of the required degree or diploma)
• Valid Driver’s License
• Automobile Insurance
• Basic Life Support Certification
• 2 years recent relevant experience

Skills & Knowledge

• Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues and dynamics of grief
• Knowledge of Medicare, accrediting bodies, State and all other regulations applicable to Bereavement programs
• Knowledge of community resources available to bereaved persons
• Excellent computer skills
• Ability to discern sensitive issues and respond appropriately
• Sensitivity to the human needs and situations encountered in the provision of hospice and bereavement care
• Detail-oriented and creative
• Excellent organizational skills, interpersonal skills, problem-solving abilities, and ability to prioritize work
• Ability to work effectively with a minimum of supervision
• Ability to work under time-pressure and changing priorities, and to deal with potentially conflicting demands
• Professionalism when interacting with clients and co-workers
• Ability to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts
• Ability to effectively plan, organize, prioritize work, and solve unique problems as they arise or identify when to consult supervisor
• Ability to deal with a challenging work environment from time demands and conflicting priorities
• Self-directed learning to meet professional development and/or license requirements
• At orientation and annually thereafter, must be able to meet core competencies for position and area of specialty, as appropriate

Schedule

• Full Time (40 hours per week)
• Shift: Days, 8 hours per day
• Days of the week: Monday - Friday
• Weekend requirements: As Needed

Pay

Pay Range is $48.39 to $67.75 per hour. The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program.
