Responsibilities

• Provide comprehensive behavioral health services in an ambulatory clinic setting under the general supervision of the CMC Medical Director.
• Perform psychosocial assessment, individual and family therapy, treatment planning, coordination of care, and linkage to community resources.
• Support behavioral and medical integration by collaborating closely with primary care providers and interdisciplinary teams to promote whole‑person care.
• Assist patients in managing mental health conditions, life transitions, trauma, and chronic disease‑related behavioral changes while maintaining compliance with state licensure, payer requirements, and MGRMC policies.

Requirements

• Master’s degree in counseling, Social Work, or a related behavioral health field from an accredited institution.
• Current Arizona license as LPC, LCSW, or LMFT (independent level).
• Minimum of one (1) year of outpatient or integrated care experience.
• Current CPR Certification (AHA – Healthcare Provider).
• Arizona Department of Public Safety fingerprint clearance card (or eligibility to obtain).
• Proficiency in electronic health record (EHR) systems and basic computer literacy.
• Excellent written and verbal communication skills and ability to maintain composure under stress.
• Demonstrated ability to collaborate effectively with medical providers, families, and community partners.

Qualifications

• Experience in an integrated primary care–behavioral health model.
• Familiarity with Meditech or similar EHR system.
• Knowledge of CPT/DSM-5/ICD-10 diagnostic and billing processes.
• Two or more years of clinical practice in rural health or community behavioral health.
