Behavioral Health Support (Montgomery County)

About the role

The Starting Point program offers support to individuals in the unique model of Psych Rehab, serving those with a mental health diagnosis, to reach their own, personally identified goals. This includes helping individuals find and maintain employment, get involved in community activities, establish healthy habits, find housing, and more.

Responsibilities

• Build rapport and effectively engage with individuals about their life goals and barriers to recovery
• Drive to and meet with individuals in homes and community locations
• Collaborate with the individual being served and the Starting Point team to develop, implement, update and review treatment plans that are recovery-oriented, strengths-based, and person-centered
• Collaborate with the individual being served to develop successful strategies for identifying, achieving, and maintaining goals in four key areas: living, learning, working, and socializing
• Maintain accurate and current case documentation in an electronic health record
• Support individuals to find and use resources and supports in their community

Requirements

• Education/Certification: Bachelor's degree required, preferably in a related field such as psychology, social work, or human services OR Certified Peer Specialist (CPS) certification required OR CPRP (Certified Psych Rehab Practitioner) certification required
• Experience: Some prior work or internship experience related to mental health services (preferred)
• Driving Requirements: A valid US driver's license for at least two years, not including time with permit. No serious traffic convictions within the past three years. (Serious convictions include, but are not limited to: DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations and/or at fault accidents within that period.) Access to a reliable vehicle during work hours. Vehicle must have valid registration and inspection stickers. Valid auto insurance. You will be asked to provide proof that you are a covered driver on the policy.

Work Schedule

Monday- Friday 9am-5pm or 8am-4pm (With flexibility on evenings required based on the needs of individuals served)

Essential Working Conditions/Physical Demands

• Work in program sites and the community routinely providing direct care and/or support with individual(s)
• Stand, sit or run by self, or to provide proper care and supervision with an individual
• Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations
• Able to assist an individual who has fallen, regardless of weight or size
• Walk, climb stairs by self, or to provide direct care and/or support
• Work in a standard office environment and utilize desktop computer equipment daily
• Able to lift or move items associated with sedentary and light work (exert up to 20 lbs.)
• Travel to offsite locations and regional office locations as necessary
