Role: Behavioral Health Specialist Responsibilities Deliver integrated behavioral healthcare to adult Medicare patients in community-based primary care settings.Perform comprehensive assessments and develop person-centered treatment plans.Provide individual and group therapy using evidence-based approaches such as CBT and motivational interviewing.Collaborate with primary care providers, nurses, and care teams to address mental health issues like depression and anxiety, and support behavior change related to chronic conditions.Coordinate referrals to community resources to enhance patient care.
