Behavioral Health Specialist

Responsibilities

• Deliver integrated behavioral healthcare to adult Medicare patients in community-based primary care settings.
• Perform comprehensive assessments and develop person-centered treatment plans.
• Provide individual and group therapy using evidence-based approaches such as CBT and motivational interviewing.
• Collaborate with primary care providers, nurses, and care teams to address mental health issues like depression and anxiety, and support behavior change related to chronic conditions.
• Coordinate referrals to community resources to enhance patient care.
