Behavioral Health Specialist

Role: Behavioral Health Specialist Responsibilities Include Conducting comprehensive behavioral health assessments for adults on Medicare within an integrated primary care setting.Providing short- and long-term evidence-based therapy tailored to patient needs.Collaborating closely with primary care providers, nurses, and care teams to develop coordinated treatment plans.Managing a diverse caseload, supporting patients with chronic and complex conditions, and responding to crises.Contributing to quality improvement initiatives aimed at enhancing patient-centered, preventative care in underserved communities.
