Behavioral Health Screener (LMSW/ LCSW)

Job Summary

Community Healthcare Network is seeking a Full-Time Behavioral Health Screener to join their team.

Responsibilities

• Provides diagnostic assessments and screening services to individuals interested in behavioral health services.
• Uses appropriate scoring tools to identify baseline for provision of care.
• Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy.
• Proactively refers patients to a different level of care.
• Counsels on a daily basis with BH Regional Supervisors to discuss clients screened and where to assign.
• Utilizes Motivational Interviewing (MI) to assess readiness for BH services.
• Billable documentation is locked within 24 hours.
• Maintains a small caseload of 10 patients, discussing cases in clinical supervision.
• Adequately schedules patients for psychiatric, behavioral health, or other services.
• Uses evidence-based models of treatment with patients on caseload (e.g., Cognitive Behavioral Therapy, Eye Movement Desensitization and Reprocessing, Interpersonal Therapy, Seeking Safety, Dialectical Behavioral Therapy, Problem Solving Therapy, Behavioral Activation, etc.).
• Performs other related duties, as assigned.

Requirements

• M.S.W. Degree.
• Licensed Master Social Worker or Licensed Clinical Social Worker.
• Bilingual Spanish strongly preferred.
